SIM card PUK-locked

Hi devvers:

I did a totally stupid and noobish thing last night and tried to secure my SIM card and set up a password. Apparently there was already a password. I tried the 3 strikes and you're out thing, so now I'm locked from my phone. Is there any way I can get by this without taking it into T-Mo? I'm afraid to, since I have Cyanogen installed and that could possibly void my contract. Help would be appreciated and thanks in advance.

*Edit: By the way, if I call T-Mobile and they give me my PUK code to enter, how do I enter it in Cyanogen Mod? On the Lock screen, it doesn't have anywhere to enter the PUK code. All I see is a button for emergency calls. Please help.

Basically do this: Touch the Emergency call button at the bottom/middle of screen of your phone and enter this with the keypad(you may have to call or do an online chat with T-Mobile first to get your PUK code) ---------------------------->

**05*<PUK Code>*<enter a new pin>*<confirm the new pin>#

Example input: **05*123456789*1234*1234#

sim locks are a little extreme for security... i think a lock screen combination should be sufficient. (try that so you dont end up in a similar situation)

there is no way to obtain the PUK w/o calling your service provider? correct?

is there a program similar to superoneclik to unlock the phone? similar to the samsung captivate galaxy S.

I obtained my PUK code and tried the method above, but it tells me it is incorrect. Any input?
